Committee for the Protection of Human Subjects (IRB Schedule)

All UH faculty, staff, or students who plan to conduct research involving the use of human subjects must have approval from the University of Houston Committee for the Protection of Human Subjects (UHCPHS) prior to the initiation of any research project. Projects requiring approval include, but are not limited to:

  1. retrospective chart reviews
  2. analysis of archival data
  3. prospective studies utilizing data from patients routine vision examinations, existing tissue samples, in addition to other clinical and basic research involving human subjects, and
  4. research conducted at institutions (e.g., Other universities or hospitals, NASA) other than UH where UH faculty participate as investigators.

The College of Optometry also has a human subjects research committee.
NOTE: All applications, revisions, and renewals with revision must be reviewed by the Human Subjects Committee at the College of Optometry prior to their submission to the UHCPHS. Renewals without revision and Final Reports should be submitted directly to the UHCPHS; review at the College level is no longer required. 
The College Human Subjects Committee strongly encourages all new investigators and those with limited experience in human subjects research to take the on-line training course offered by the University.

Instructions for ICON Submission and College Review

 

  1. Complete the application in ICON but do NOT “submit”. Leave your application in “pre-submission” status.
  2. Click on the “manage ancillary review” link on the left side of the screen, click the “add” button, and enter “optometry” as the “organization”.  Select Review Type as “Department” from the drop down, and Select “Yes” to the question asking if a response is required. This will send an email to the College Human Subjects Committee alerting us to an application. Do NOT add Optometry as an ancillary reviewer until you are ready for the College Committee review.
  3. When the College Committee has completed the review, the PI will receive an email from ICON if revisions are necessary and the comments that need to be addressed will be available in ICON.
  4. If revisions are required, make the revisions in ICON. The PI must AGAIN go to “ancillary review” and enter “optometry” as the “organization”. If  you do not re-enter the  “organization” after making the revisions, the College Committee will NOT be alerted to re-review your application.
  5. If no (additional) revisions to your submission are necessary, the College Committee will approve your application in ICON. Currently, ICON does not email the PI when an ancillary review is approved… so for now expect an email from a College Committee member (most likely Karen Fern) stating we have approved the application. At some point in the not too distant future, ICON will be updated to send the PI an email from ICON notifying you of the approval.
  6. Once the College Committee has approved the submission, Laura Frishman needs to approve the application. Under “ancillary review” enter “Laura Frishman” in the “person” field, select “Department” from the review type drop down, and click “Yes” for whether a response is required. This will alert Laura and she will approve the submission. DO NOT enter Laura as a person when you enter “optometry” as the organization (during step 2 and 4). Laura Frishman should only be entered in the ancillary review section AFTER the College Committee has provided approval. If you enter both Optometry AND Laura Frishman at the same time, Laura will receive the alert prior to the College Committee review. Laura will not know when the College Committee has completed the review—this will delay your approval from Laura Frishman and your submission to CPHS. Currently, ICON does NOT notify the PI when Laura has approved your application.
  7. When both the College Committee and Laura Frishman have approved your application, the PI clicks the “submit” button to send  the application to CPHS in ICON. You will be able to see if Optometry and Laura Frishman have approved the application for submission by looking in the ancillary review section. NOTE: If you click the “submit” button before approval is received from both the College Committee and before Laura Frishman has approved your submission, , CPHS will send the submission back to the PI for the required reviews/approvals as a part of CPHS pre-review process. Failing to obtain the appropriate reviews/approvals prior to submitting to CPHS will delay your submission and it is possible the submission will be returned to the PI after the College submission deadline. This will likely result in a 1 month delay in review of the application by CPHS.

Do NOT click submit until both the College Committee and Laura Frishman both show in the ancillary review section as having approved the application!
